The TAIHS Diabetic Care Program aims to improve the health of clients with diabetes through education and care management strategies that improve a person’s diet, physical activity and weight management. The program also aims to enable a client’s self-management of their chronic illness.
Working closely with the Doctor and the Chronic Care Coordinator, clients can access the following diabetes services:
- Full health check, assessment, screening and testing including blood sugar levels, HbA1c, urine ACR and cholesterol
- Foot checks and eye health checks
- Diabetes education and advice about nutrition
- Care planning and ongoing medical advice
Additional service and group
TAIHS also invites clients to join the Got Sugar Diabetic Group. The group meets on a regular basis, they do fun activities around healthy cooking and they share recipes and their own personal experiences with regard to managing their health care.
